Essential Services
The Practice will provide, within core hours, primary medical services required for the management of its registered patients and temporary residents, who are, or believe themselves to be ill with conditions from which recovery is generally expected, terminally ill or suffering from chronic disease; delivered in the manner determined by the Practice in discussion with the patient.
The Practice will include the provision of appropriate ongoing treatment and care to all registered patients and temporary residents, taking account of their specific needs including the provision of advice in connection with patient’s health, including relevant health promotion and the referral of other NHS services.
Additional Services
In addition to the Essential Services provided by the practice, the practice also provides the following Additional Services:
- Childhood Vaccinations and Immunisations
- Flu and Pneumococcal Immunisations
- Minor Surgery
- Fitting IUCD
- Monitoring for patients on long term medication
- Anti-coagulation monitoring
- Neonatal examinations
- Post natal examinations
- Dispensing for some patients at Armitage surgery – details on request at reception
- Rheumatology bloods monitoring
Chronic Disease Management Clinics
If you have one of the following conditions you will be invited annually to attend a Nurse led clinic.
- Diabetes
- Asthma
- Chronic Obstructive Pulmonary Disease
- Hypertension
- Hyperthryoidism
- Renal Impairment
- Epilepsy
- Mental Health conditions
- Learning Disabilities
- History of a heart attack or heart bypass surgery
- History of a CVA (stroke)
Run by our fully trained Nursing team, these clinics offer various appropriate clinic tests, advice, support and a general health check.
Health Promotion
The Horsefair Practice Group strive to support its patients in areas of health promotion: offering services to help with weight loss, smoking cessation and general healthy lifestyle advice. Also, if you are aged between 16 and 75 and have not had a consultation for at least 3 years, you are entitled to receive a general health check. Patients over 75 can also request a general health check annually.
Minor Surgery
Most of the doctors perform a limited range of minor surgery. Please book an appointment with a GP for more specific advice.

Child Health and Child Immunisations
Regular sessions are held for all childhood immunisation.
Mondays: 2.00pm – 4.00pm : Armitage Clinic
Immunisations carried out by the Practice Nurses are performed during this clinic. The routine 8 week child health surveillance checks are performed by a doctor at this clinic.
Thursday: 10.00 –12.00 noon : Horse Fair Practice at our Sandy Lane facility
Immunisations carried out by the Practice Nurses are performed during this clinic. The routine 8 week child health surveillance checks are performed by a doctor at our Horse Fair Surgery during this session and again a Health Visitor is available.
